One of the most common questions people ask before starting a GLP-1 medication is also one of the most honest: what happens if I stop? Will the weight come back? Will I go back to feeling hungry all the time? What does stopping actually look like?
The answers are not what most people want to hear, but they are important to understand before starting therapy, while you're on it, and especially if you're considering stopping. The short answer

SURMOUNT-4 followed a similar design with tirzepatide. Participants first completed 36 weeks of tirzepatide treatment, losing an average of 20.9% of their body weight. They were then randomized to either continue tirzepatide or switch to placebo for another 52 weeks.
Among those who stopped tirzepatide, weight rebounded by an average of 14 percentage points over the 52-week follow-up period. Those who stayed on the medication lost an additional 5.5% during the same period. In practical terms: participants who stopped tirzepatide regained roughly half their lost weight within one year, while those who continued kept losing.
Worth noting: even with significant weight regain, many participants who stopped tirzepatide still maintained some net weight loss compared to their starting weight before the study. Not all the progress was lost. But the trend was unmistakably toward regain once the medication was discontinued.
The pattern with liraglutide is consistent with what was seen in the semaglutide and tirzepatide trials. In the SCALE Obesity and Prediabetes trial, participants who completed 56 weeks on Saxenda and then stopped regained a substantial portion of their weight over the following 12-week off-drug period.
